The Site is operated by Union Square Dental Practice (“Union Square Dental,” “we,” “us,” or “our”). By accessing or using the Site, you agree to these Terms. If you do not agree, do not use the Site.
- Informational Purpose Only
The Site provides general information about Union Square Dental, dental services, oral health topics, office policies, and ways to contact the Practice. Site content is not a substitute for an examination, diagnosis, treatment plan, or advice from a licensed dentist or other qualified healthcare professional who has evaluated your individual circumstances.
No dentist-patient relationship is created merely because you visit the Site, read its content, send a message, request an appointment, or submit information for a virtual consultation. A dentist-patient relationship is established only as permitted by applicable law and Practice procedures.
- Emergencies and Urgent Conditions
The Site and its forms are not monitored continuously and must not be used for medical or dental emergencies. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department. For an urgent dental concern, call the Practice at (415) 693-9139. Do not rely on a website submission for time-sensitive care.
- Appointment Requests and Virtual Consultations
When you use an appointment or virtual-consultation form:
- Your submission is a request only and does not create or confirm an appointment.
- An appointment is confirmed only after the Practice communicates confirmation to you.
- Any preliminary information provided through a virtual consultation is limited by the information and images submitted and may change after an in-person examination, diagnostic imaging, or other evaluation.
- You are responsible for providing information that is accurate, current, and complete and for having authority to submit photographs or information relating to another person, including a minor.
- Submit only the information requested. Do not send Social Security numbers, payment-card details, or other information the form does not request.
Electronic communications can be delayed, misdirected, or accessed by unauthorized persons despite reasonable safeguards. If you have not received a response within a reasonable time, call the Practice.
- Financial, Insurance, and Payment Information
Any information on the Site concerning fees, insurance, financing, membership plans, or payment options is general and may change. Coverage, benefits, reimbursement, and patient responsibility are determined by applicable plans, payors, contracts, and treatment details. Nothing on the Site guarantees insurance coverage, reimbursement, financing approval, treatment cost, or a particular outcome.
Payment and patient-registration links may direct you to independent third-party services. Those services operate under their own terms and privacy notices. Review those terms before providing information.
- Permitted Use

This authorization does not permit public advertising use of identifiable patient information or photographs without any additional authorization required by law.
- Third-Party Services and Links
The Site may contain links to, or embedded features from, third parties such as maps, videos, accessibility tools, patient-registration services, payment services, security tools, and social-media platforms. Third parties control their services, and their terms and privacy practices apply. A link or embedded feature does not mean Union Square Dental endorses or controls the third party. We are not responsible for third-party availability, content, security, or practices.
- Privacy and Health Information
Our Website Privacy Policy, explains our online information practices. Information that constitutes protected health information may also be governed by the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), other health-privacy laws, and Union Square Dental's Notice of Privacy Practices. The Website Privacy Policy does not replace the Notice of Privacy Practices.
- No Warranties
TO THE MAXIMUM EXTENT PERMITTED BY LAW, THE SITE AND ITS CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE.” UNION SQUARE DENTAL DISCLAIMS ALL EXPRESS OR IMPLIED WARRANTIES RELATING TO THE SITE, INCLUDING WARRANTIES OF ACCURACY, COMPLETENESS, TIMELINESS, MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, SECURITY, AND UNINTERRUPTED OR ERROR-FREE OPERATION. We do not warrant that Site content will reflect the most current medical or dental information or that the Site will be free of harmful components.
- Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, UNION SQUARE DENTAL AND ITS OWNERS, DENTISTS, EMPLOYEES, CONTRACTORS, AND AGENTS WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR LOSS OF DATA, PROFITS, OR USE, ARISING FROM OR RELATING TO THE SITE OR THESE TERMS, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. Nothing in these Terms excludes or limits liability that cannot lawfully be excluded or limited, and nothing limits rights or remedies arising from professional dental services where such a limitation is prohibited.
- Indemnification
To the extent permitted by law, you agree to defend, indemnify, and hold harmless Union Square Dental and its owners, dentists, employees, contractors, and agents from third-party claims, damages, and expenses arising from your unlawful misuse of the Site, your violation of these Terms, or your infringement of another person's rights. This section does not apply to the extent a claim results from Union Square Dental's own negligence, professional services, or conduct for which indemnification may not lawfully be required.
- Changes, Suspension, and Termination
We may update, suspend, restrict, or discontinue any part of the Site at any time. We may revise these Terms by posting an updated version and changing the effective date. Your continued use of the Site after an update means you accept the revised Terms to the extent permitted by law.
- Governing Law and Venue
These Terms are governed by the laws of the State of California, without regard to conflict-of-law principles. Subject to any rights that cannot lawfully be waived, a dispute relating solely to use of the Site or these Terms must be brought in a state or federal court with jurisdiction in San Francisco County, California. This provision does not alter any separate rights, obligations, or procedures applicable to dental care or a dentist-patient relationship.
- Severability and Entire Agreement
If any provision of these Terms is held unenforceable, it will be enforced to the maximum extent permitted and the remaining provisions will remain in effect. These Terms and the Website Privacy Policy constitute the agreement between you and Union Square Dental regarding use of the Site, but do not replace agreements, consents, financial policies, treatment documents, or notices applicable to dental services.
